Adding Survey and Inspection Responses

You can add responses to a survey or inspection from MobileWorx, giving you the ability to provide feedback and complete necessary inspections while on-the-go.

How to Add Survey and Inspection Responses

  • On the Mobile Dashboard, tap Add New Survey/Inspection.
  • Tap Choose Survey/Inspection Template.
  • Filter using the tabs or enter search criteria at the top of the screen, if needed. Then, select the Survey/Inspection Template you would like to respond to.
  • Tap Done in the top right corner.
  • Tap Choose Worker if you are rating the work of a staff member.
    • Select the name of the staff member from the list. You can use the Search bar to narrow down the list of staff members, if needed.
    • Tap Done in the top right corner.
  • Tap Choose Location. Every survey or inspection requires a location to be selected in order to record a response.
    • Begin typing in the Search bar at the top to populate the list. Tap on the Location for this survey or inspection.
    • Tap Done in the top right corner.
  • The Are You Rating Work of A Staff Member? toggle is typically used if this is a work quality inspection or a satisfaction survey about a specific work order.
    • Tap on the toggle and use the Choose Worker option to define the staff member, or skip this option if you would like MobileWorx to remind you to attribute it to a staff member later when you're entering the survey results.
  • Tap Done in the top right corner to begin entering responses.



  • Enter the responses by either tapping on the Pass or Fail buttons or selecting the appropriate rating if the survey/inspection uses a rating scale.
  • Tap the Save Where I'm At button to save the survey/inspection at any time and return later to complete it.



  • To save time, you can tap the Pass For All or Fail For All buttons to mark all responses as either Pass or Fail.
  • Tap the menu icon on a question to reveal additional actions you can take.
    • Comment - Tap this link to add comments to this response. Tap Save to add the comment to the survey/inspection.
    • Corrective Action - Depending on how the survey/inspection template was created you may see a Corrective Action link. This lets you initiate a request for work into any application and relate it back to that question. You may also be prompted to create a work order based on a template, or create an alert for this service area. *Note: The request will not be generated until you mark the entire survey as complete.
  • To submit the survey/inspection responses, tap Complete Survey.
